First Visit

When you arrive for your first visit, please arrive 10 minutes prior to your scheduled time in order to allow us to set up your patient file and begin your dental treatment in a timely manner.

As a new patient, your initial exam and cleaning will last approximately one hour. If you do not have current x-rays, they will need to be taken. Oral hygiene instructions will be provided along with suggestions to help you care for your teeth. If necessary, we will outline your existing dental problems and prepare a proposed treatment plan.

Before your arrival, please click on the "New Patient Forms" link below, and complete the online forms. Please do not select the SUBMIT button until you have completed the information on all pages. Use the arrows at the top and bottom of the form to navigate it. Once you arrive at our office, we will ask you to attach a signature to the completed forms.

If you have current x-rays (bitewings within the last two years; panoramic or full mouth series within the last five years) at another dental office, please print the "Release Authorization Form" below, complete and mail it to your previous dental office.

If you are not familiar with the HIPAA privacy act, please find attached a Notice of Privacy Practices for your review.
